How do you get a GLP-1 prescription in Miami?

Dr. Linda Moleon, MDJuly 21, 2026

Key takeaways

  • • Getting a prescription for one of these medicines in Miami is a sequence, not a single appointment: intake and history, a clinician evaluation, an eligibility determination, then a pharmacy or delivery arrangement.

  • • The clinical criteria come from FDA-approved labeling, not from Miami-Dade County or from any individual local clinic, so they are the same in Little Havana, Doral, Miami Beach, and Homestead.

  • • Most Miami residents now begin through telehealth, and the evaluation itself is the same clinical review whether it happens on video or in an exam room.

  • • Expect to be asked for a documented height and weight, a full medication list, and your personal and family medical history, and expect specific laboratory follow-up once you are on therapy.

  • • Some of the medicine names that show up in Miami searches are still in research and cannot be prescribed anywhere in the United States.

  • • This is general education about a process, not personal medical advice, and no article can tell you whether you qualify.
  • Dr. Linda's take

The Miami version of this question usually arrives with a little frustration attached. You have seen the billboards on the Dolphin, heard the ads on Spanish-language radio, walked past the med spa on Biscayne with a sign in the window, and you still cannot tell what the actual first step is or who is allowed to say yes. That confusion is not your fault. Local marketing has run far ahead of local explanation, and a lot of what looks like a shortcut here turns out to be a detour.

So let me lay out the sequence plainly. The statewide picture, the eligibility background, and the coverage basics belong to a separate, broader piece, and I would rather point you there than repeat it. What follows is the Miami layer on top: what the steps look like when you live here, what gets asked of you along the way, and the two logistics problems that are genuinely different in a subtropical city.

What actually has to happen before a prescription exists?

Four things, in order, and none of them can be skipped.

The first is an intake. You provide a health history, a current medication and supplement list, and a documented height and weight rather than a remembered one. The second is an evaluation by a clinician licensed to practice in Florida. That person, not a form and not an algorithm, is the one who decides. The third is an eligibility determination, where your history is measured against published clinical criteria and against anything in your record that would rule the medicine class out for you. The fourth is the fill, meaning the prescription is routed to a pharmacy you can actually reach or arranged for delivery to your address.

If you want a structured way to organize your own history before step one, the eligibility quiz walks through the same categories a clinician will ask about. It is a preparation tool, not a decision.

Who decides whether you meet the criteria, and what are they?

A licensed clinician decides. What that clinician measures you against is public, and it is worth knowing before you walk in.

Semaglutide for weight management is indicated, alongside a reduced calorie diet and increased physical activity, to reduce excess body weight and maintain weight reduction long term in adults with obesity, and in adults with overweight who also have at least one weight-related comorbid condition. In the trials supporting tirzepatide's approval for weight management, participants had obesity, defined as a body mass index of 30 or higher, or overweight, meaning a body mass index of 27 to under 30, plus at least one weight-related comorbid condition such as dyslipidemia, hypertension, obstructive sleep apnea, or cardiovascular disease, and the statewide background to those thresholds sits in our guide to getting started in Florida.

Notice what that means practically. A weight-related condition can move someone into eligibility who would not qualify on body mass index alone, which is why a thorough intake asks about your blood pressure, your lipids, your sleep, and your blood sugar rather than only about the scale. Notice also what it does not mean. Meeting a published criterion is a necessary step, not a guarantee, since the clinician still has to weigh the rest of your record.

Does a Miami evaluation happen online or in person?

For most people here, it happens online, and that is a reasonable outcome rather than a lesser one. In a metro area where a fifteen mile trip can take an hour, a video evaluation removes the single biggest reason people abandon the process partway through. The clinical substance does not change with the format. The same history is taken, the same red flags are screened for, and the same licensed person signs the prescription.

In-person still matters in specific situations: if you need a physical exam element that cannot be done remotely, if you have a complex cardiac or gastrointestinal history that your existing Miami specialist should weigh in on, or if you simply want a local relationship. Many people end up with a hybrid, using telehealth for the evaluation and their existing primary care office for blood work.

Both product labels list a personal or family history of medullary thyroid carcinoma, or multiple endocrine neoplasia syndrome type 2, as a contraindication, a screen that does not change from one South Florida provider to the next, and how to compare those providers is covered in our Miami and South Florida guide.

What documentation and laboratory work usually get requested?

Bring three things and you will move faster: a recent documented height and weight, a complete medication list including anything you take for thyroid function or blood sugar, and what you know about your family's medical history. That last item surprises people, and the contraindication above is the reason for it. The intake is asking about your relatives, not only about you.

On the laboratory side, the labels direct clinicians to monitor blood glucose before starting and during treatment in patients with diabetes, to monitor renal function in patients reporting reactions that could lead to volume depletion, and to monitor patients with a history of diabetic retinopathy for progression, and there is a fuller picture of what tends to get checked and when in our guide to labs before and during treatment.

One useful thing to know so you are not alarmed by an absence: routine monitoring of serum calcitonin or the use of thyroid ultrasound is described in the labeling as being of uncertain value for early detection of medullary thyroid carcinoma. A clinician who does not order that particular test is following the label, not cutting a corner.

Why do Miami searches for retatrutide lead nowhere?

This one deserves a direct answer, since it is among the most searched terms in this city and among the most misleading.

Retatrutide is not an approved, prescribable medicine in Miami or anywhere else in the United States. The peer-reviewed literature describes it, alongside other newer receptor agents, as still in investigation or development. The weight results circulating on social media come from a phase 2 trial involving adults who had a body mass index of 30 or higher, or a body mass index of 27 to less than 30 plus at least one weight-related condition. Phase 2 is a research stage, not a pharmacy shelf.

If a local clinic tells you it can supply that molecule today, treat it as a reason to walk away rather than a reason to hurry. The same skepticism belongs to any offer that skips the evaluation step entirely.

What changes about pharmacy pickup and delivery in Miami?

Heat, mostly, and here it is a real variable rather than a footnote.

These are refrigerated products with defined limits. Semaglutide pens and syringes are stored in a refrigerator at 2°C to 8°C, which is 36°F to 46°F, and if needed the pen can be kept from 8°C to 30°C, or 46°F to 86°F, for up to 28 days before the cap is removed. Tirzepatide labeling says to protect the medicine from heat and light, and that a single-dose pen or vial can be stored unrefrigerated at temperatures not to exceed 30°C, or 86°F, for up to a total of 21 days, a limit worth reading next to what South Florida heat does to storage.

Now put those numbers against a July afternoon here, where a parked car interior or an unshaded doorstep passes eighty-six degrees easily. Two practical consequences follow. Ask how a delivery is packed and whether it requires a signature, and plan a pickup that goes from the pharmacy counter to a home refrigerator without a long errand in between.

    The bottom line

There is no Miami-specific rulebook for who qualifies, and that is genuinely good news, since it means nobody here can gatekeep you with a rule that does not exist. What is local is everything around the decision: how you reach a licensed clinician without losing a day to traffic, which offers in this market are worth ignoring, what your intake should have ready, and how a temperature-sensitive medicine survives the trip from a pharmacy counter to your refrigerator in August. Handle those four and the process stops feeling like a maze, and the question shifts from where do I even start to what do I want to ask.
