Does Medicare Cover Ozempic, Wegovy & Zepbound? (2026)

Dr. Linda Moleon, MDJuly 4, 2026

the same drug can be covered or excluded depending on why it is prescribed. here is the 2026 map.

the short answer
Standard Medicare Part D cannot cover any GLP-1 prescribed only for weight loss; that is federal law. It does cover specific GLP-1s for other approved uses: Ozempic and Mounjaro for type 2 diabetes, Wegovy for cardiovascular risk, and Zepbound for sleep apnea. For weight loss, the new Medicare GLP-1 Bridge covers Wegovy, Zepbound, and Foundayo for about $50 a month starting July 1, 2026.

coverage is decided by the diagnosis, not the drug

The same GLP-1 can be covered for one condition and excluded for another. Medicare looks at why the medication is prescribed. Here is how the common drugs land in 2026.

| Drug | Part D may cover it for | Not covered for |
| --- | --- | --- |
| Ozempic (semaglutide) | type 2 diabetes | weight loss |
| Mounjaro (tirzepatide) | type 2 diabetes | weight loss |
| Rybelsus (oral semaglutide) | type 2 diabetes | weight loss |
| Wegovy (semaglutide) | heart attack and stroke risk in adults with cardiovascular disease who are overweight or obese | weight loss alone, use the Bridge |
| Zepbound (tirzepatide) | moderate to severe obstructive sleep apnea in adults with obesity | weight loss alone, use the Bridge |

the three approved doors into Part D

If you have type 2 diabetes, a diabetes-labeled GLP-1 is typically covered, often with prior authorization. If you have established cardiovascular disease along with excess weight, Wegovy may be covered to lower heart risk. If you have obstructive sleep apnea with obesity, Zepbound may be covered for that. Each of these is a separate, legitimate path, and many people qualify through one without realizing it.

oral options exist too, not only injections.

for weight loss, the Bridge is the path

Beginning July 1, 2026, the Medicare GLP-1 Bridge covers Wegovy, Zepbound (KwikPen), and Foundayo for weight management at a flat $50 a month for eligible Part D members. It runs through December 31, 2027 and works outside your normal Part D plan. We cover it in full in our Medicare GLP-1 Bridge guide.

what you will actually pay

For a diagnosis-based prescription in 2026, your Part D plan applies its deductible (up to about $545), after which your annual out-of-pocket spending on covered drugs is capped at $2,100. Through the Bridge, the math is simpler: $50 for a 30-day supply. Compounded versions are not covered by Medicare in either case.
